The survey found that about one in three U.S. adults has cut back on other household spending to pay for healthcare or medicine.

Gallup said people reported reducing spending on a wide range of essentials and routine costs. Those cutbacks included cutting back on utilities, driving less to save on gas money, stretching prescription doses, and borrowing money to cover care.

The findings add to a longer trend in affordability research from Gallup and West Health. Their earlier studies also found that healthcare expenses pushed many Americans to cut back on food, over-the-counter medicine, clothing, and leisure spending.

This time, the message is broader than skipped doctor visits. The latest results suggest healthcare bills now shape how people handle ordinary living expenses, even before they think about long-term savings or future goals.

Many Households Say Medical Bills Delay Major Milestones

Gallup also found that healthcare costs are changing how Americans plan their lives. In a separate nationally representative survey of 5,660 U.S. adults conducted from October to December 2025, respondents reported that medical expenses had delayed a major life event or personal goal in the past 4 years.

Those delayed plans included buying a home, taking a vacation, changing jobs, moving, getting married, or having children, according to the Gallup report. The survey framed healthcare costs as a force that reaches far beyond hospitals and insurance paperwork.

That broader impact matters for health coverage because financial stress can shape physical and mental well-being. When people delay major decisions, limit daily spending, or borrow money for treatment, the burden often extends into stress, instability, and reduced access to future care. This last point is an inference based on the survey’s reported financial strain and the well-established public health link between financial stress and health behavior.

The Gallup release also warned that 2026 could bring even more pressure. It pointed to higher insurance premiums, rising out-of-pocket costs, the expiration of some Affordable Care Act subsidies, and planned cuts in Medicaid enrollment as factors that may leave millions more exposed.

Low-Income Adults Face the Sharpest Pressure, but Not Alone

Gallup and West Health have repeatedly found that low-income and uninsured adults face the heaviest strain. In a 2021 survey, 35% of adults in households earning under $24,000 said they could not afford needed care in the prior year. That study also found that 12% of U.S. adults cut back on food to pay for healthcare.

Yet the new findings suggest this problem now reaches deeper into the middle class. Gallup said even middle-income households report making financial trade-offs to cover medical bills, showing that the affordability problem no longer rests solely at the bottom of the income scale.

The organization also released state-level rankings in 2025 that showed a sharp divide between states with better and worse affordability experiences. Residents in the bottom 10 states were substantially more likely to make tradeoffs, including stretching prescriptions or borrowing money, than residents in the top 10 states.

That pattern suggests geography still shapes the pressure people feel. Insurance rules, local care costs, and access to support programs can all influence how often households face these healthcare cost tradeoffs. This is a reasoned conclusion drawn from Gallup’s state-level comparison.

Healthcare Affordability Now Affects Health and Behavior

The health impact of these tradeoffs can be direct. When people stretch prescriptions, delay care, or borrow money to stay in treatment, they may increase their risk of worsening illness or unstable medication use. Gallup and West Health have already documented that tens of millions of Americans forgo needed care or prescriptions because of cost.

That makes the new survey more than a story about budgeting. It shows that healthcare affordability can simultaneously alter daily behavior, future planning, and medical decision-making. Those overlapping pressures help explain why affordability remains one of the most persistent public health problems in the United States.

West Health said in earlier work that 112 million Americans were struggling to afford healthcare, while 93% felt what they paid was not worth the cost. The newest Gallup findings suggest that the problem is no longer confined to delayed appointments or isolated debt. It now affects how people live, spend, and plan.

For health systems and policymakers, the takeaway is clear. Rising medical costs do not just influence treatment choices. They also shape household stability, prescription adherence, and major life decisions, making healthcare cost tradeoffs an issue that reaches well beyond the clinic.

New trade-offs in healthcare costs are reshaping how Americans manage their budgets, according to new West Health-Gallup data.
