Recent polling from KFF found that health care costs will affect both turnout and vote choice for large shares of registered voters, especially Democrats and independents.

The issue is not abstract. It is showing up in household budgets first. New surveys from Gallup and West Health found that about one in three Americans made at least one daily life tradeoff in the past year to pay for health care. Common responses included prescription rationing, borrowing money, and cutting other spending. (Gallup.com)

That strain appears to be helping push health care higher on the affordability agenda. A recent KFF analysis said health care affordability is now top of mind for many Americans and has risen above other necessities in its polling.

Rising Insurance Costs Are Driving the Pressure

One major source of frustration is insurance. The end of enhanced Affordable Care Act tax credits has left many marketplace enrollees facing much higher costs in 2026. KFF reporting says 80% of people who re-enrolled in ACA coverage this year reported higher premiums, deductibles, or other costs than last year, and 51% said those costs were “a lot higher.”

Those increases are forcing new tradeoffs. More than half of returning ACA enrollees said they were cutting or expecting to cut food, clothing, or other household basics to keep up with health expenses. Around 17% said they were not confident they could continue paying premiums for the full year.

The pressure extends beyond the ACA market. Gallup and West Health say millions of Americans are also delaying major life decisions because of medical costs, including job changes, home purchases, retirement, and even plans to have or adopt a child. That suggests health care costs are shaping not just consumer behavior, but long-term family planning.

Why the Politics of Affordability Are Shifting

KFF’s January poll found that more than three-quarters of Democratic and independent voters say health care costs will influence both whether they vote and which party they support. Among Democrats, about two-thirds said the issue would have a major impact on their 2026 vote.

That makes health care different from a narrower insurance policy debate. It now overlaps with broader economic anxiety. When voters think about affordability, they are not only thinking about groceries, rent, or gas. Many are also thinking about monthly premiums, surprise bills, prescription costs, and whether coverage will actually protect them when they need care. This is an inference drawn from the polling pattern across KFF and Gallup data.

Recent coverage of ACA enrollees underscores that point. KFF reporting said about three-quarters of ACA enrollees who are registered voters expect health care costs to affect their vote in 2026. That helps explain why affordability debates are no longer separating general cost-of-living concerns from health policy.

A Pocketbook Issue With Broad Health Consequences

The political importance of the issue rests partly on its real-world impact. When households stretch prescriptions, skip meals, or drop insurance to stay afloat, the result is more than financial stress. It can also mean delayed treatment, worse chronic disease control, and greater pressure on emergency care. That is a public health inference, but it follows directly from the spending tradeoffs reported in recent surveys.

For now, the clearest signal is that the health care affordability voters care about is moving closer to the center of economic politics. Polling and household data both suggest that many Americans now see medical costs as a daily affordability problem, not just a specialized policy topic. As the 2026 campaign season continues, that shift may shape both the debate and the electorate.

Health care affordability concerns are becoming a more visible political issue as costs rise across insurance, prescriptions, and routine care.
